What is a PACE program?
PACE (Program of All-Inclusive Care for the Elderly) is a national model of care that provides older adults and their caregivers innovative health care and support.
The PACE model is based on the belief and experience that older adults with chronic care needs are better served living in their homes and communities, whenever possible.
