At Summit ElderCare, you’ll have one team in one place providing medical care, advice, coordination and support.
When your loved one joins Summit ElderCare, he or she will receive individualized care and services by a team of health professionals who specialize in geriatric care and work together to meet your loved one’s needs.
Individualized care plan
Once your loved one joins the program, he or she will be evaluated by the interdisciplinary care team. The team will work together with you and your loved one to assess his or her needs and create a care plan that is customized to help him or her remain safely in the community..
A team of health care professionals that works together
The Summit ElderCare team includes many elder care professionals with different areas of expertise. Team members include primary physicians, nurse practitioners, nurses, social workers, rehabilitation staff, activities coordinators, health aides, home care specialists and much more. Consultations with specialists are also included based on the individual participant’s needs.
- All Summit ElderCare staff have expertise in geriatric care (care of older adults).
- Your loved one’s care team meets frequently to discuss his or her care and progress.
- You’ll have on-going access to the care team, including 24-hour coverage for emergency situations.
Easy access to your care team right at our PACE adult day health centers
When your loved one needs medical care, our staff is here to help. Your loved one can get most of his or her care (including medical visits, routine lab work, medical treatments and services like physical therapy) on-site at our PACE adult day health centers. The familiar setting of the PACE center helps reduce some of the anxiety that may be associated with visiting multiple doctors’ offices and hospitals—and you’ll have less worry about transportation.
Your loved one will be cared for by staff who know him or her and who have access to his or her care plan and electronic medical record. If your loved one needs monitoring or follow-up care, our staff will coordinate it.
