What makes our PACE adult day health centers unique?
Most adult day health centers offer social activities, meals and some nursing, rehabilitation and social services. Our PACE adult day health centers also include on-site medical care with access to your loved one’s primary care physician and other PACE staff. The entire interdisciplinary care team is also available on-site.
At our PACE adult day health centers, your loved one can:
- Socialize and make friends

Participate in a variety of recreational activities such as Wii games, computers, crafts, outdoor recreation and entertainment, and more.
- Receive rehabilitation therapy
- Get nutritional counseling
- Access social services
- Enjoy hot lunches and snacks
- See their team of on-site doctors, nurses and other healthcare professionals.

Summit ElderCare also contracts with the following adult day health programs:
- St. Francis Home Adult Day Health Center at 101 Plantation St. in Worcester, Massachusetts
- Dodge Park Day Club at Dodge Park Rest Home in Worcester, Massachusetts
If your loved one attends the program at St. Francis Home Adult Day Health Center or Dodge Park Day Club, they’ll receive their medical care and rehabilitation at one of our PACE adult day health centers.
We’ll work with you and your loved one to develop an individualized care plan. Adult day health center services are based on the level of services your loved one may need, your preference, and the availability of day service slots.
Medical transportation
If your loved one needs transportation to and from our PACE adult day health centers or off-site medical appointments, we can help. Our transportation providers work closely with you and your interdisciplinary care team to address your loved one’s unique needs.
