Summit ElderCare, a Program of All-Inclusive Care for the Elderly (PACE), bridges the gaps between participants’ needs for medical care, supervision and a feeling of independence and caregivers’ needs for support, respite and centralized care for their loved one.
Participants in Summit ElderCare benefit from having care coordinated by a team of health care professionals with experience and training in geriatrics. We specialize in caring for people with Alzheimer’s and dementia.

We keep the participants’ and the families’ unique needs in mind to find creative and practical solutions that help elders continue to live at home, instead of in a nursing home.
