Summit ElderCare Centers

Medical care and social activities

When you join Summit ElderCare, you get medical care, social support and health insurance. And you also become part of the community at your Summit ElderCare Center.

As a participant, you receive medical care at your Summit ElderCare Center—and adult day health services if you choose. Your community may also offer other options for adult day health services.

Benefits of the Summit ElderCare Centers

In addition to providing medical care, our Summit ElderCare Centers offer a way for you to:

One location for care

As a participant, you visit your Summit ElderCare Center for your medical appointments. That means you receive consistent care. Your team works with you and your family to develop a plan for your care. And you enjoy the benefit of receiving centralized care from health care professionals with expertise in geriatrics, or the care of older adults.

A caring community

Your Summit ElderCare Center offers you the opportunity to socialize with fellow participants, volunteers and staff. Although it is a personal choice, we recommend that participants attend our adult day health program at least once a week.

When your care team sees you on a weekly basis, they get to know your current health status and can more easily help prevent and mitigate future issues. In the process, they get to know you as a person, too.
