Our complete and coordinated services will help you care for your loved one so that he or she can live at home, not in a nursing home.
Summit ElderCare is a complete medical, insurance and social support program for older adults who qualify for nursing home care. Our team of social workers, nutritionists, rehabilitation specialists, home health care specialists, doctors and nurses who specialize in geriatric care, and other professionals oversee your loved one’s care at home and at our PACE adult day health centers. Our services include full coverage for prescription drugs and medical transportation*, resources for caregivers, and much more.
Medical care
A team of doctors, nurses, specialists, social workers and therapists works together to help take care of your loved one. Our Summit ElderCare team will meet with you and your loved one to coordinate the individualized medical services that may be needed.

PACE adult day health centers
At our PACE adult day health centers, we offer medical care, nutritious meals, rehabilitation services like physical therapy, the opportunity to make new friends, and a variety of stimulating activities tailored to meet the needs and interests of program participants.

Insurance protection

Home care
If your loved one needs help at home, our Summit ElderCare team will design and arrange a package of home care services tailored to his or her individual needs. Some examples of services are: help with medications, bathing and grooming, meal preparation and homemaker services.
