Information for referral sources
Eligibility
To be eligible for Summit ElderCare, the participant must be:
- Be age 55 or older
- Live in the Summit ElderCare service area (includes Worcester County and the communities of Hudson and Marlborough)

- Meet the Medicaid nursing facility clinical eligibility criteria.
This means that applicants must require enough assistance to require nursing home care.
- Be able to live safely in the community as determined by the Summit ElderCare interdisciplinary team.
- There are no financial eligibility requirements to join Summit ElderCare.
- Although Fallon Community Health Plan sponsors Summit ElderCare, participants do not need to be FCHP members to enroll.
We can help you determine whether or not an individual is eligible.
Cost
We accept Medicare, Medicaid and private pay. For people eligible for Medicaid, there may be a monthly “spend-down” due to financial criteria. Even if you do pay a monthly spend-down or premium, the cost is less than a nursing home and includes so much more. Participants and caregivers also have the peace of mind of knowing that the premium or “spend-down” amount won’t increase if there’s a change in the participant’s health.
