Eligibility

Grandfather with his grandson playing video gamesTo be eligible for Summit ElderCare, applicants must:

  • Be age 55 or older
  • Live in the Summit ElderCare service area (includes Worcester County and the communities of Hudson and Marlborough)
    See a map and list of towns »
  • Meet the Medicaid nursing facility clinical eligibility criteria. This means that applicants must require enough assistance to require nursing home care.
  • Be able to live safely in the community as determined by the Summit ElderCare interdisciplinary team.

Are there any financial requirements?

There are no financial eligibility requirements to join Summit ElderCare.

Do you have to be a Fallon Community Health Plan member to be eligible for Summit ElderCare?

Although Fallon Community Health Plan sponsors Summit ElderCare, you do not need to be an FCHP member to enroll.

Once your loved one is enrolled, he or she won’t need other medical or prescription coverage since Summit ElderCare will provide the insurance protection (including Medicare Part D prescription coverage).
