Rebecca Alexander, M.S.N., FNP-C
Photo of Rebecca AlexanderRebecca Alexander is a Certified Family Nurse Practitioner at Summit ElderCare in Worcester at the Grafton St. location. Her nursing experience includes certification as a Certified Wound Ostomy Continence Nurse Specialist (WOCN). She received her master’s degree as a Family Nurse Practitioner from Stony Brook University School of Nursing, and has worked in private physician practice, home care and in long-term care settings.

 

Tanya D. Campus, M.D.
Tanya Campus MD photoDr. Campus is a family and geriatric medicine clinician who brings both home care and nursing home experience to the Summit ElderCare team. She is a graduate of the Jefferson Medical College of Thomas Jefferson University in Philadelphia and completed her residency at Maine Dartmouth Family Medicine in Augusta, Maine. Dr. Campus completed her geriatric fellowship there. Before joining Summit ElderCare, she was the medical director of Sebasticook Valley Home Care and practiced at the Pittsfield Health Care Center, both in Pittsfield, Maine. Dr. Campus sees Summit ElderCare participants at the Leominster location.

Nicole Cornelius, A.N.P., G.N.P.
Nicole CorneliusNicole Cornelius has served as a Nurse Practitioner for a community medical office and as an R.N. for a home health agency, and also provided nursing care to adult-medical-surgical-oncology patients in an area acute care hospital. She holds a Masters degree from the University of Massachusetts in Worcester Graduate School of Nursing and is both a certified Adult Nurse Practitioner and Gerontological Nurse Practitioner. Nicole Cornelius primarily cares for participants associated with the Summit ElderCare site in Charlton.

Ashraf F. Elkerm, M.D.
Dr. Ashraf F. Elkerm is a graduate of Alexandria University in Alexandria, Egypt, where he received his medical degree and master of surgery degree. He completed a residency at Saint Vincent Hospital in Worcester, where he served as chief medical resident. Dr. Elkerm has more than 20 years of experience in clinical practice and research at hospitals in Egypt, Saudi Arabia and the United States. Dr. Elkerm maintained a private practice for 10 years serving the needs of a large panel of elderly patients. He is an assistant professor of medicine at the University of Massachusetts Medical School in Worcester, and is board certified in internal medicine. Dr. Elkerm mainly practices as a primary care internist at Summit ElderCare in Leominster.

Neal Fallis, M.D.
Dr. FallisDr. Fallis is an internist with extensive experience in caring for geriatric patients. Dr. Fallis obtained his medical degree at Boston University Medical School and completed an internship in pathology at St. Mary Medical Center in Long Beach, California, followed by a residency at Long Beach Veterans Hospital. Prior to joining Summit ElderCare’s medical team, Dr. Fallis practiced internal medicine for over 27 years at a Westborough medical office. He is board certified in internal medicine and primarily sees Summit ElderCare participants at the Charlton location.

David R. Fields, M.D.
Dr. FieldsDr. Fields is a board-certified geriatrician, clinician and educator. He obtained his medical degree from the Medical College of Virginia and completed his residency in internal medicine at the Bronx Municipal Hospital, Albert Einstein College of Medicine in Bronx, N.Y., and held a fellowship in geriatric medicine at Harvard Medical School at Brigham and Women’s Hospital in Boston. Prior to joining Summit ElderCare’s medical team, Dr. Fields practiced at UMass Memorial Medical Center, where he was the Clinical Chief in the Division of Geriatrics. Dr. Fields mainly sees participants at Summit ElderCare’s Grafton Street location in Worcester.

Tinevimbo Jaravaza, M.D.
Dr. JaravazaDr. Jaravaza received her medical degree from St. George's University School of Medicine in Grenada, West Indies, and completed her residency in internal medicine and fellowship in geriatric medicine from Wayne State University/Detroit Medical Center in Michigan. As a board certified geriatric physician on the Summit ElderCare medical team, Dr. Jaravaza provides comprehensive medical care, including chronic disease management, acute care and preventive care. Dr. Jaravaza primarily practices at Summit ElderCare's East Mountain Street location in Worcester.

Judith E. Lengieza, M.S., R.N.-C.
Photograph of Judith LengiezaJudith Lengieza is a certified nurse practitioner at Summit ElderCare. Judy is responsible, in coordination with the primary care physicians, for providing primary and acute medical care for the program participants at all Summit ElderCare sites. Judy’s 20-plus years of experience as a nurse include adult primary care, preventive health care, urgent care and care of frail elders. She received her Masters of Science degree in Nursing from Simmons College, Boston, and her Bachelor of Science in Nursing at Marquette University, Marquette, Wis.

Ruth M. Miller, G.N.P.
Ruth Miller brings more than 25 years of experience in geriatric nursing disciplines, including psychiatry, adult sickle cell care, hematology and oncology to Summit ElderCare. Ruth received her Masters of Public Administration from the University of North Carolina in Greensboro and her bachelor’s degree in Gerontology/Administration at High Point University, High Point, N.C. Ruth primarily cares for Summit ElderCare participants at the site on East Mountain Street in Worcester.

Kathy Pulda, RN, A.N.P.
Kathy PuldaKathy Pulda is an Adult Nurse Practitioner who cares primarily for patients associated with Summit ElderCare in Leominster. In this role, she provides primary and acute medical care to program participants in coordination with the primary care physicians. Kathy brings more than 30 years of experience. She has served as a guardianship nurse, director of the homemaking program for Jewish Family Services in Worcester, and as a nursing supervisor in long-term care. Ms. Pulda has also held positions as a surgical staff nurse and nurse unit manager. Kathy received her Master of Science in Nursing at the University of Massachusetts Graduate School of Nursing in Worcester.
