Worcester, Mass., November 4, 2008―Summit ElderCare®, a Program of All-inclusive Care for the Elderly (PACE), announces the appointment of David R. Fields, M.D., as a member of the Summit ElderCare medical team at a new Summit ElderCare location on Grafton St., in Worcester.
An internal medicine physician with extensive experience in geriatrics, Dr. Fields will provide comprehensive medical care to Summit ElderCare participants, including chronic disease management, acute care and preventive care. He will work with other Summit ElderCare geriatrics team members to provide coordinated care and services that allow participants to remain in their own homes and avoid nursing home placement.
Dr. Fields comes to Summit ElderCare from UMass Memorial Medical Center, where he served as the Clinical Chief in the Division of Geriatrics.
A resident of Worcester, Dr. Fields obtained a bachelor’s degree in economics from the University of Virginia at Charlottesville, and received his medical degree from the Medical College of Virginia. He completed his residency in internal medicine at the Bronx Municipal Hospital, and completed a fellowship in geriatric medicine at Harvard Medical School.
He is board certified in internal medicine and geriatrics, and currently serves as an assistant professor of medicine at UMass Medical School.
