This year, Summit ElderCare marks it's 10th anniversary, underscoring a program committed to helping elderly citizens keep their independence. Summit ElderCare is the only program of its kind in Central Massachusetts.
Fallon Community Health Plan, Summit ElderCare's sponsor, is a recognized leader in providing health care solutions to Medicare-eligible individuals. In fact, Fallon Community Health Plan sponsored one of the first demonstration projects in the country. This became Fallon Senior Plan in 1980. Today, over 30,000 individuals are enrolled in Fallon Senior Plan health plans.
In 1995, recognizing a need for more comprehensive services, care and support for elders who want to remain independent -- and for their caregivers -- founded Elder Service Plan, known today as Summit ElderCare. Summit ElderCare is a Program of All-inclusive Care for the Elderly (PACE), one of six in Massachusetts. It was provided permanent provider status by the Centers for Medicare & Medicaid Services in 2002.
The PACE model evolved through the belief and experience that older adults with chronic care needs -- and their families and caregivers -- are better served by living in their homes and communities whenever possible.
