Summit ElderCare appoints Margaret S. Schwab, R.N. as Clinical Nurse Manager
Worcester, Mass. July 9, 2007
Summit ElderCare, a Program of All-Inclusive Care for the Elderly (PACE) sponsored by Fallon Community Health Plan, announces the appointment of Margaret S. Schwab, R.N. as Clinical Nurse Manager of a planned Summit ElderCare site, which is currently under construction in Leominster and scheduled to open this summer. In this position, Schwab will oversee the daily activities of providing nursing care to Summit ElderCare participants, including the management of all nurses and nursing health aides. She will also collaborate with the Summit ElderCare on-site primary care physician and other interdisciplinary team members to ensure that all planning and care delivery processes are in compliance with delivery processes coordinated to meet a program participant’s needs.
Schwab has more than 20 years of experience as a nursing professional, including care in emergency, intensive care, social service and case management areas. She comes to Summit ElderCare from Brigham and Women’s Hospital in Boston. Previously, Schwab spent 20 years at Athol Memorial Hospital in Athol, Mass., and held the position of nurse manager.
A resident of Athol, Schwab holds a diploma of nursing from the Peter Bent Brigham School of Nursing in Boston, and an associate’s degree from Mount Wachusett Community College.
Summit ElderCare is sponsored by Fallon Community Health Plan, and is one of six PACE programs in Massachusetts and more than 40 programs nationwide.
