Eligibility

  • Be age 55 or older
  • Live in the Summit ElderCare service area
  • Meet the Medicaid nursing facility clinical eligibility criteria.
  • Be able to live safely in the community as determined by the Summit ElderCare interdisciplinary team.

Service area

Summit ElderCare is available to adults in Hampden County, Worcester County and the communities of Easthampton, Granby, Hudson, Marlborough, South Hampton and South Hadley. We have five PACE centers: Charlton, Leominster, Springfield and Worcester.
